We have a patient that came in for removal of an infected sebaceous cyst. It was quite a large, extensive excision and the patient will require more than 10 days follow up for repacks. I have never run into this before with what to do after the 10 days global is over. At this point the nurses are doing all of the repacks so would I be able to charge a 99211 after the 10 days or should he start to make an appointment with the doctor for any further packings? This patient is HAMP commercial insurance.

Thanks for any help!! :)

The nurses can do the repacks and you can bill the claim as "incident-to" the physicians service once the global period has expired. If there is no specific code for the repack, then you can use 99211. All the "incident-to" guidelines must be followed though.
