A&P:
Stage IA squamous cell carcinoma of the right lung, s/p lobectomy, to continue surveillance. Clinically there is no evidence of recurrence.

Do I use history of lung cancer as primary or is there a code for surveillance?
 
Follow-Up

I always look to see how long this patient has been disease free. If it has been long than 5 years, see you guidelines for Follow-up coding in the front of the book. first code used is Z08, followed by History code. the patient would be NED, no evidence of disease. If less than 5 years, just use the cancer code.
