I am struggling with understanding the difference between monitoring, ie, 'referencing labs/other tests' and evaluation, ie, 'test results'.

Most often we have documentation that looks like this, BG-125/25 or HgbA1 done 9/16/2021 10.0 or lipid panel 8/2021 reviewed, or GFR> 100 LFT's abnormal.

To me, it is very hard to decipher the difference between, the monitoring- 'referencing the labs/tests' and the evaluating-"test results."

Both descriptors, for each portion of M.E.A.T. , seem so close to the same thing.
Any tips on deciphering which are a referencing of tests/labs and which are just test results would be fantastic! Thank you in advance.
