Wiki wheelchair needs assessment

We are a family practice who saw a patient for a wheelchair needs assessment. This was the only service provided that day. I have tried to bill it with just the code, however our billing software says there must be a modifier attached but I am unsure what modifier to attach. Can anyone help me?
