Wiki when coding 95180 and the patient has a reaction...what to code

When a patient comes in for 95180.....does well for two hours then has a reaction that the Dr. has to give meds for and stay by the patients side for alteast an hour to monitor vitals and get the patient stable.....what would you code for that hour. My Dr. is trying to code a 99215 then a 99354. Any help would be greatly apprciated.
