Please bear with me, I am not an OB coder.
At our facility we do only prenatal and post natal care; no delivery.

My office is having a disagreement about when the 59425/26 codes start. Are there always three office visits charged before the pregnancy codes start? What if a patient comes to the 1st visit with a positive urine test, then the office orders an ultrasound (done the next day), and at 2nd appointment, the U/S and positive pregnancy are discussed. Shouldn't the 59425/26 code start at the second visit?

Antepartum

When billing antepartum care, the first visit that starts the flow sheet is what should be included in the 59425/6 code. When we bill for antepartum care, U/S are billed at the time of service, and do not count as a visit toward the 59425/6 code, UNLESS the patient was seen on the same day by a provider.

I recommend visiting this blog for more details on billing for antepartum.
