For example if the HPI of a visit is a level 2, the exam is a level 3 an d the medical decision making is moderate, what code should be assigned to the visit?

If it is a new patient the lowest of the three key components will determine the level. If it is an established patient you are allowed to use the higher 2 so you may throw out the lowest and then the lowest of the remaining 2 will determine the level. for your example.
