When to use 796.9 over 796.4 in a clinical laboratory setting?

A laboratory will not assign a disease category to an abnormal lab test. The decision was made to assign a sign or symptom code. If there is no listing for a specific test, the defaulat is 796.4. Effective 10/01/2009, a new code of 796.9 was added. My question is under what circumstances would this code be used in a clinical laboratory setting?
