which is the way has the highest reimbursement for physician

My doctor has done a breast reconstruction with a tram flap, he had another physician in the practice do one side and he did the other, what is the best way to bill this with highest reimbursement for each physician and the most correct way, Thank you guys so much for your help any ideas would be appreciated.
