Will Medicare pay CPT 11406 if the tumor is benign?

Is a 2nd ICD-10 code required to show the patient meets the Medicare LCD requirements. For example the patient complained of pain

11406 is specifically for a benign lesion. You shouldn't need a secondary diagnosis. Are you getting a denial for not medically necessary? What dx code was it billed with?
