Can someone help me with finding documentation to show providers that it is illegal to withhold treatment by cancelling appointments because the patient hasn't made payments on their balance?
 
To say this is illegal is probably a stretch. Unwise is more likely in my opinion. When I was managing a practice we formally terminated some patients for nonpayment or for chronic missed appointments. Consult your state medical society and your professional liability carrier for guidelines. In general you must notify the patient and give them time to arrange other care. Otherwise you risk being sued for patient abandonment.
