can anyone guide me as to where I would find in writing Words Trump numbers on a dx. this is for an audit challange.

For example:The doctor documents: BP 160/90 but never documents the word(s) "elevated blood pressure" or "hypertension" we cannot code it.

Not documented, not coded.
