If the provider documents an injury, but does not specify where it occurred, is it okay to assume that it is NOT work or accident related? Or does one have to query the provider (and if the provider doesn't know, call the patient to find out...)?
 
Definetly inquire the provider. It will save you the hastle in the long run of denials, having to refund the wrong payers, and so on.. I would not bill this out until you have the provider claify this, and also make the corrections to the documenation.
