Hi,

When coding office E/M visits if the doctor has a Working Dx can that be used as the dx or would I still just go with the signs and symptoms?

Working dx cannot be coded for physicians. Any verbiage that indicates uncertainty--possible, probably, differential, need to confirm, etc, you will code the signs/symptoms.
