Wound care provider states" Mechanical debridement of biofilm to the level of subcutaneous tissue with a Vashe soaked debri lolly and dried well" wound size is 2 cm x 4 cm, what would be the most accurate CPT code? 97597 or 97602 or something else?
 
I’d go with 97597 here. Even though they mention using a Vashe-soaked debrilolly, that’s still considered selective mechanical debridement, not non-selective. They’re specifically removing biofilm and devitalized tissue, which fits 97597 perfectly. Since the wound is 2 cm x 4 cm (8 sq cm), it falls under “first 20 sq cm or less”, so you’d code 97597 only. 97602 would only apply if it was truly non-selective (like wet-to-dry or enzymatic debridement), which isn’t what’s being described here.
