If a provider states in documentation that a patient has diabetes but the wound being treated is not stated as a diabetic wound, do you still code this as a diabetic wound because the patient does have diabetes? Are there some guidelines somewhere that someone could point me to?

Specifically answering a question about nephropathy not specifically linked to DM, the AHA gave the instruction to code them separately. The explanation was that without a stated link, no link could be coded.

Since this provider is not linking the wound to DM, you cannot assume that link yourself.
