A patient comes in the ER with abdominal pain. They order a CT of the Abdomen and CT of the pelvis. The pelvis is clear but the abdomen shows nephrolithiasis. Can i use 592.0 diagnosis with both or just the abdomen since it didn't show up in the pelvis CT ?

I would use Abdominal Pain as your main diagnosis, then add the Nephrolithiasis diagnosis second. I would point the second diagnosis to the abdomen CT scan CPT code and point the main diagnosis to the pelvic CT scan. I hope this helps.
 
i would just code 592.0 and not abdominal pain since abdomen pain is a sign/ symptom of nephrolithiasis ;)
 
thickening nasal sinus

How to code "mucosal thickening paranasal sinus"?
should I code it as sinusitis? (is thickening is a hypertrophy?)
 
X ray diagnosis

You always code the finding as the principal diagnosis followed by signs and symptoms.

Since there wasn't any conclusive findings on the pelvis you would code the abdominal pain first as the reason for the scan followed by the 592.0 as a secondary code.
