Can anyone point me in the right direction for information on documentation requirements needed in the body of x-ray reports? I am based in Louisiana, and it is my understanding that the body of the report needs to state the numer or types of views obtained, along with the radiologists' findings (just stating it in the header or exam type is not sufficient enough); however, the radiologists are requiring valid "proof" that this information is required. Please, any help is appreciated.
