Have the guidelines for Yag cap after cataract surgery changed? Is this still a once a lifetime procedure? If patient had cataract surgery on 8/5/25 but then comes back within the 90 day global of the surgery and needs a yag cap, we can bill this new procedure with a 78 modifier? I am getting conflicting answers on this matter.
