I am looking for some insight/clarification on the use of Z01.411. We have used it when the provider discovers an abnormal finding during her annual wwe that the patient was unaware of, like a cervical polyp, breast lump, vulvar lesion, etc. Now, under new management, we are being advised to use it if a patient is seen for her annual wwe with hrt refill and the rx is adjusted due to minor symptoms, adding N95.9 also. Did something change in the guidelines for use of this icd-10 code? We are just trying to understand its intended use so we can assign it accurately. Any info is much appreciated.
 
According to Coding Clinic, "an "abnormal finding" is a newly discovered condition, or a known/chronic condition that has increased in severity, (e.g., uncontrolled, and/or acutely exacerbated)"

If the Rx is not working as expected, I think it would fall under "uncontrolled" and the Z01.411 would be appropriate
