The guidelines say that Z51.11 can be first-listed or primary if the encounter is 'chiefly' for the administration of medication to treat neoplasm.

What defines as 'chiefly'? If the patient is seen after treatment by the provider?
 
For example, if the codes being billed are for antineoplastic treatment. If it's for a physician visit, I wouldn't use Z51.11. Instead I'd use the chief complaint/primary reason for the visit (usually the cancer that's being treated for example) for the visit codes. Also keep in mind that there is a NCCI edit with chemo admin codes and the office visit codes.
