Wiki Z71.0 for Advanced Care Planning

Hi!

Can I bill CPT code 99497 using the ICD code Z71.0? The provider documented that Advanced Care Planning was discussed.

You can use CPT 99497 or 99498 per minutes treated but add the chronic issues or dx codes pt dealing with and then last dx code Z71.0. Also discuss type of medical form which can be done alone and face to face then add dx Z71.0 last. But you can add Eval mgnt code to same claim if on same date and discussing care & treatment of chronic dx but link dx codes or chronic conditions to the Eval Mgnt codes..
