Hi I am new to HCC coding- Dr. is seeing a pt with h/o DVT- pt still with treatment of Warfarin and Eliquis- however DR. reported D68.69(Secondary hypercoagulable state NOS).
It was my understanding D68.69 was to be used related to Afib -
Is is correct to use D68.69 related to DVT as well or would it need to change to Z79.01(Long term (current) use of anticoagulants)?

This dx. is used for coagulation in the blood and so is dx R79.1 or D68 as first dx code then add Z79.01 last. Different lab test can cover differ disease, the CPT manual in lab section gives you more data too. But the provider ordering the lab test done will tell you what his or her reason is for testing=medical necessity.
