How are you coding a mammographic guided breast biopsy. Are you using CPT 19499? Payor denied saying CPT 19081 is the correct code but this is with stereotactic guidance. Thank you PROCEDURE: The procedure with its risks of bleeding...

I'm not saying that this note in particular I was told is wrong, just apparently for a lot of Ortho codes for hand procedures that I've coded with F modifiers the insurance is denying because they want LT/RT and so now I'm second guessing myself...

I don't know what EMR you use, but we use Epic and it allows multiple NDCs per line. Previously, we were told one line per NDC but when we do that it gets denied, so we nest the NDCs. EDIT: Wait, Rachel is that you? O.O

You can code for ORIF and cubital tunnel release. 29838 is the only other code you can code. I'd consider a -22 modifier, BUT the operative note is so weak and crappy it does not justify a -22 modifier. Your surgeon is leaving money on the table...

Hi, yes we have many residents. You can definitely bill whatever the appropriate code is with a GC modifier. There must be statement (as you noted) that the TP was present for the procedure and signed by TP.

This is a common tactic by F&A orthos who use a ton of BMAC. If you document that you extracted "spicules of bone" along with your aspirate, it qualifies for a bone graft code typically and does not get kicked out. That strategy still works for...

NRaizman replied to the thread Question Revision LRTI.
1: This is a -shamefully poor- operative note. Someone, hopefully you, should tell your surgeon that he or she is documenting the work -so- poorly that it is leaving money on the table. 2: What your surgeon fails to document is opening up the...

The provider performing the supervision can be virtual. The MD doesn't have to be in the office if the APP is providing the virtual supervision for the RN. My comments about scope have to do with the service being provided under virtual...

Share the text of the new direct supervision rule with your administrator. Effective Jan. 1, virtual DS it is an option for all services that do not have a global surgery indicator of 010 or 090...

Who is telling you that you are doing it wrong? I have not read the note but F modifiers would be preferred in this situation rather than -RT.
