Industry Notes:

Use This Form For Clerical Reopenings

If you want a clerical error reopening of a claim from HHH MAC NHIC, you should use the form NHIC has created for that purpose, the MAC says in a message to providers. "The purpose of a clerical error reopening is to correct a minor error or omission from the original claim submission, outside of the Medicare appeal process," NHIC explains in a review of reopenings and appeals. "All reopenings are granted at the discretion of the contractor." More appeals information and a link to the clerical reopening form are at www.medicarenhic.com/PA/parta_appeals.shtml.
