Neurology & Pain Management Coding:

Antisocial and Asocial Are Synonymous for This Dx

Question: Encounter notes indicate a patient has an “asocial personality.” Is this the same as borderline personality disorder?

RCI Subscriber

Answer: No, but it is the same as antisocial personality disorder (ASPD), which you’ll report with F60.2 (Antisocial personality disorder). Beneath the entry for F60.2, you’ll see the following alternate terms:

  • Amoral personality (disorder)
  • Asocial personality (disorder)
  • Dissocial personality disorder
  • Psychopathic personality (disorder)
  • Sociopathic personality (disorder)

What is it? People with antisocial personalities tend to be oblivious to (or not care about) certain social norms, especially when it comes to other people. They often lack empathy and remorse, which can lead to dishonest and deceitful behavior. “Patients with ASPD may be witty, flattering, and manipulative one minute and argumentative, angry, or arrogant the next. They believe they are superior to others and steal, tell lies, and fight,” according to AAPC’s Codify.

Remember Excludes notes: There are two Excludes notes for F60.2:

  • Excludes1: conduct disorders (F91.-)
  • Excludes2: borderline personality disorder (F60.3).
