Alabama Subscriber Answer: You should code a rectovaginal fistula as 619.1 (Digestive-genital tract fistula, female). You won't find any specific CPT for the biopsy of the fistula, so you may have to opt for a vaginal biopsy (57100-57105 as being the closest, or a biopsy of the rectum (45100, Biopsy of anorectal wall, anal approach [e.g., congenital megacolon]).
