Use a Total Knee Arthroplasty Revision Code for a Poly Revision

Question: An orthopedic surgeon performed total knee arthroplasty revision where they placed a patella button and performed a polyethylene revision. What codes should I use to report the procedure?

Answer: You’ll assign 27486 (Revision of total knee arthroplasty, with or without allograft; 1 component) appended with modifier 52 (Reduced services) to report the patella button and polyethylene revision. Modifier 52 demonstrates that the surgeon replaced the patella button and exchanged the polyethylene component instead of inserting an entirely new implant.
