The physician orders states fracture of the wrist, but the x-ray states normal three view wrist.
The patient does not have a fracture so what do I code as a diagnosis?

Signs and symptoms that led him to believe that the arm might have been fractured.

Or use the V71.x codes then the symptoms secondary.

I would call the referring physician for signs and symptoms if you do not have any.
