I would like to know what we would need for documentation for a dr flushing ears for
an doctor or a nurse, to support a 99212 for the doctor and a 99211 for the nurse??
I want to make sure we do this right, Thank you,
 
If the doctor only gave treatment for the impacted cerumen then use
69210 Removal of impacted creumen this is for both ears

If the doctor indicated in his notes that he gave treatment for other complaints in addition
to the impacted creumen than use the correct visit code with a modifier 25

example 99213 25 401.9 250.00
69210 dx for impacted creumen

I was told we could only charge 69210 if an instrument was used to remove cerumen; that if irrigation was used them only E/M code?? :confused:
