Extensive right external iliac, right common femoral, right profundus femoris and superficial femoral thromboendarterectomy with 2 bovine patch graft insertions and bovine patch angioplasty.

Patient is 100 days post uncomplicated carotid endaraterectomy, now with severe right lower extremity ASO w/ claudication.

Procedure above performed....

Any insight on what all to capture for this procedure?

Approach: longitudinal incision made over the common femoral artery below the inguinal ligament.

I would bill 35372 for the Profunda Femoral and 35351 for the External Iliac. Dx: 440.21.

Now, per Dr. Z you can bill per vessel treated, however, these lesions MUST be non-contiguous to code for more than one. If you feel that based on the dictation that each vessel was separate and non-contiguous, you can bill for 35371 as well with a modifier 59.
