What are the CPT and modifiers for the following case:

70 yr old with pvd and premature CAD
From a right femoral approach, both the left and right coronary systems was accessed and visualized, the left ventricle was accessed and visualized. Left ventriculogram was performed. Closure device was deployed. The patient's coronary anatomy is right dominant.
