I am trying to get feedback from anyone who may know how this particular procedure should be coded? Is there a specific code for this description? I can't seem to locate one. Any suggestions would be greatly appreciated.

There still doesn't appear to be one. See this older thread with guidance from the AMA. I've searched around too, and can't find anything more current. I've seen other guidance (not from trusted resources) that say to use 64640, but lumbar sympathetic nerves are not peripheral nerves, so I don't agree with that.
