Can a physician get credit for documenting a negative response for an HPI element?
EXAMPLE: patient has "no modifying factors" or patient has "no associated signs and symptoms"?

Does anyone know of documentation in regards to this?

The HPI is from the patients statements. If the patient states negative responses to the questions then they would still be applicable to the bullets points for the HPI.
