Does anyone bill 17110 (destruction) with bleomycin? Do other bill 11900 (intralestional injection) with bleomycin? Just wondering what the rationale is or appropriateness of using the different cpt codes. Any input would be greatly appreciated!

Maybe 96405, since this is a chemotherapy drug, not a substance with destructive properties.
