You will have to be more specific. What are you trying to accomplish by adding a modifier?

The most likely modifier is QW (CLIA Waived Lab Test), but that's not the only possible option. I'll need more info to help you further.

We have a Type 2 CLIA certification and our G0479 is being denied by Medicare stating that it needs to be a higher certification (4). The question is, if we bill G0479 can we use modifier QW and not place the CLIA we have on the claim? Thanks!

I would advise to use the QW modifier and still place your existing type 2 CLIA number on the claim. The reason they say you need a higher CLIA certification is because you do not have the QW. With the QW, type 2 CLIA waiver is sufficient.
