Hi,

The patient is admitted with shortness of breath. The patient is diagnosed with COPD exacerbation. The physician stated in discharge diagnosis the patient has COPD exacerbation secondary to acute URI infection. Now what would be PDX in this case ????.

According to Coding and Reporting Guidelines:
J44.0, "COPD with acute lower respiratory infection" when the patient has both. Here is a snip of the AHA Coding Clinic Question:

If the provider documents COPD with pneumonia, it would be appropriate to assign code J44.0 followed by the code for the URI J06.9
The rationale has been based on the provider documenting "with" and is now interpreted to mean with/due to. The COPD exacerbation would be sequenced first with the URI secondary.

You have to remember "what brought the patient to the bed". In this case, the COPD exacerbation was treated primarily and the URI was the underlying cause of the exacerbation :)
