Can someone tell me if I can code 26356 and 64835 on separate lines, or do they need to be code once with the number of units.
 
26356 has MUE of 4 so it can go on one line with units. 64835 would be on its own line with 1 unit. 64837 is to be reported for each additional nerve (one line as well MUE is 2 units)

Thank you for your answer. Speaking of units, someone in our office just brought me a procedure that was performed and is being denied for to many units. 26145 and 26478. Each has an MUE of 6, but there were 11 tendons worked on during the surgery. How would I make the corrections for proper coding?
