Rt/Lt Heart Cath - looking for coding guidance

I have coded 93460-26, 93505-26 and 76937. Both 93460 and 93505 are being denied as "not paid separate." Can someone give me some guidance? According to Craneware, they can be billed separately...? Thank you.

Was a full diagnostic right and left heart cath performed? If so, you need to add a modifier -59 to 93460.
