What ICD 10 dx codes would you use for this?
 
no they are just saying bilateral fracture of the medial malleous

I would query the provider if you don't have any additional information - this is odd terminology. If by 'bilateral' they mean that the patient has fractures of medial malleolus of both the right and left legs, then you would code S52.51XA and S52.52XA (better terminology in this case would be bilateral medial malleolar fractures or fractures of the medial malleoli of bilateral ankles). But if they are saying there is a 'bilateral' fracture of a single malleolus, then I agree with the post above - this doesn't make sense and your provider needs to give you additional information - you can't have a single fracture of the medial side of an ankle that is also bilateral.
