We are having a debate in our office. The ICD 10 guidelines and CDC are telling us 2 different things.
Im curious who is adding exposure ( Z20.828 ) to every pt getting tested for Covid regardless of known exposure or symptoms.



1) Contact/Exposure Category Z20 indicates contact with, and suspected exposure to, communicable diseases. These codes are for patients who do not show any sign or symptom of a disease but are suspected to have been exposed to it by close personal contact with an infected individual or are in an area where a disease is epidemic.

We are adding the code to patients we are testing that have been screened and determined the need to be tested, whether they are symptomatic or asymptomatic. But like I said, if they have been determined to need testing by either the RN or provider.

However, we have just started to do testing on patients who are considered high risk for being American Indian. Since the CDC and our Oregon Health Authority are saying these individuals should be tested, we are testing them. But how do i report that? They don't fall into the appropriate category for the Z20.828, and from what I have read the use of the Z11.59 is actually inappropriate to use as we are currently in the public health crisis...

I think this does fall into Z20.828. Particularly if there is a high case rate in their community.
Here is an article discussing the issue which basically says while we are in a PHE/pandemic, Z20.828 is the correct diagnosis (unless of course, the test is positive).
