I work for a small behavioral health hospital and am wondering if anyone has any advice on how to code the H&P and Psych evaluation when done by the same Physician. Are they able to use 99221 and 90792 on the same day of service? I believe that we are able to bill this if the H&P is done be the medical doctor and Psych eval is done by the Psychiatrist, but what if both are done by the Psychiatrist?
 
So are you saying I do not need a H&P and a Psych Eval for an inpatient stay? This is confusing to me, as I have been told we have to have both for an inpatient stay.

No, I'm saying the codes that make up 99221-99223 cover both services in one code. You do have to have both, but they are not necessarily separate services if they are done by one provider.
 
I code for a small psychiatric inpatient unit. Our psychiatrists typically do a 99221-99223 service as indicated by Sharon. Since our psychiatrists use the 1997 specialty guidelines, everything they would cover in the 90792 is covered in the initial inpatient visit. Our clinical social workers do an evaluation (90791), but they are not reimbursed on same day as an E&M by most payers so we have opted to set them as non-billable services for inpatient.

We generally have a medical physician perform a history and physical, and our psychiatrist does the psych evals. With our youth floor this becomes a little different, and this is my first issue with the same provider doing both services. Thanks to you ladies for pointing me in the right direction!

can you tell me if your providers document in one note, or are they putting all the info into the H&P? Our provider is questioning how to document all this information, and the fact that her Psych evals usually take her 90 minutes......any help is appreciated :)

Could you help me with a question. I understand that 90791 cannot be billed in conjunction with an e/m by the same individual. Does that mean all the providers in the same office? or just an individual? We have an LCDC billing 90791 and the MD is billing an E/M on the same date of service. Is this allow?

Our group is about to start billing for the social worker's eval (90791) in addition to the MD's eval (either 90792 or 99221-99223). Can anyone give me feedback on how it is going for your practice if you are billing this way and how you have been most successful in obtaining reimbursement?
