Wiki Simple E/M audit

Does anyone do a simple E/M audit of charts? I want to forward notes for a 99212, 99213 and 99214 to see if we are doing our note right and getting those level that we are billing. Does anyone have a per chart fee for something like this and in return can if us a detailed explanation of what we may be missing in the note and how you came to the code level ?
