If a patient had acute kidney injury several months ago and now has normal kidney function, would we still code the acute kidney injury?
 
You can only code it if the provider has indicated that the AKI condition is still present at the current encounter. If it is not clear in the documentation whether or not it is still present, or if there is conflicting information (e.g. the provider has carried forward the diagnosis but the lab results or other documentation does not appear to support it), then you would need to query the provider for clarification.
