simple question : patient came in for a 99213 , but then the doctor removed a single lesion , would that be 11106 with a 25 modifier?

Did the patient come in intending to have the lesion removed? Was it a new problem with the decision to remove made at the time of the visit? Context is all important here.

The report stated the lesion had gotten larger , Yes my mistake he came in for lesion and review medications
