Wiki Medicare reimbursement for Reclast

Our Medicare reimbursement for Reclast went down in October so we have lost money on the medication. Medicare told my supervisor that we now have to put the name and dose of the medication in box 19 for any infusion or injectable drugs to get the full reimbursement. Has anyone else seen this? I tried to search for a policy change but couldn't find it. Office is in the DC area so novitas solutions. I called them but had to hang up after being on hold for awhile.
 
I've only known that to be true of unlisted drugs, definitely not reclast. Were they able to give you information on where you can find information regarding this instruction?
