I have never coded CHF exacerbation as acute CHF, however I am getting pushback on this. How do you all code CHF exacerbation?

I was oversimplifying and didn't state it correctly. The patient has chronic diastolic heart failure with exacerbation. I have never coded this as acute on chronic because the provider does not state "acute" and there is no code for exacerbation. In my 30 years as an auditor I've never been challenged on this but now I am. Thoughts??
 
I agree with you, 'exacerbation' does not mean 'acute', and I wouldn't interpret it that way either. I would push back and ask whoever is challenging you on this to provide you with a reference.
