CKD multiple stages are documented in current headers can be coded with least stage or highest stage or unspecified ? Kindly clarify as getting different answers for this scenario...

You will need to query to provider to know which stage the patient is on the date of service.

The only guideline regarding multiple stages documented is when ESRD is documented with another stage.
