This code is being denied when billing lexiscan-78452, 93015 and A9500- I looked up Noridian website and it states per invoice. What exactly does this mean?

Hello, we did face the same issue and I hope this helps for everyone looking for this info :)

All you have to do is enter the cost of A9500 as per your invoice in 1500 claim form - either under box 19 or just above the date of service, where you list the code- as " A9500 - Invoice price - $ xx. xx"
