How do I submit medical documentation to BC/BS when an EOB is returned with an adjudication remark? Any advice would be greatly appreciated!

The CARC is CO16. We have received this CARC on claims submitted to BC/BS of Michigan, BCN, Blue Cross Complete, and BC/BS Medicare Advantage plans. We use Availity and we can't find a way to submit supportive documentation for this through the portal. Thank you so much for your help!
